Q: Why are your NoRosa products any better for my rosacea than many other things I see advertised?

A: To better understand the advantage lets first discuss what this thing called rosacea actually is. While the cause of rosacea has not been determined with certainty some of the more accepted current proposals are the following: 

Individuals with rosacea prone skin have a diminished capacity to neutralize higher amounts of reactive oxygen species (ROS) found during rosacea outbreaks. Better known as "free radicals" these need to be kept under control in order to be free from irritation and the consequence of skin inflammation that can follow unchecked ROS.

Another view as to the causation of rosacea involves the presence of Demodex mites on the skin. While a higher preponderance of rosaceans are associated with a Demodex presence, Demodex is not uncommon across the entire population at large. Some individuals manifest the rosacea symptoms while others with a Demodex presence do not exhibit symptoms of rosacea. 

A more recent development to the proposed Demodex / rosacea connection takes place within the mite itself as a bacterium in the form of  Bacillus oleronius. Recent research finds that this little critter ends up causing trouble in a big way and how this happens actually ties up with the first line about the causation of rosacea. The exudations coming from the bacteria found in the Demodex mite contain an antigen that is a potential source of irritation to the skin. And here is the third act in this play: Healthy skin has a sufficient level of auto-immunity in place to neutralize the antigen, and so life goes on without a moments notice. 

However, when this antigen makes its way on to the surface of a complexion that has a weakened capacity (rosacea prone skin) to neutralize excess ROS, this creates the setting for a series of events leading to inflammation and ultimately rosacea flaring. So, even though mites are involved as a carrier, the problem arises when the skin cannot properly provide a needed level of self immunity to neutralize the irritation that is associated with contacting an antigen arising from bacteria. 

Recent observations have shown that facial skin from patients with rosacea produces excess amounts of the serine protease kallikrein 5 (KLK5), and abnormal forms of the antimicrobial peptide cathelicidin (CAMP). It has become clear that rosacea is primarily an inflammatory and not an infectious process, and research has focused on identifying the "bad actors" in rosacea.

One of the more recent insights has to do with an entity called "cathelicidins," which have also been designated alarmins because they alarm the body that it is being attacked. Cathelicidins have some antibacterial and even some antiviral activity, but their main job is to cause an alarm manifested by inflammation. Cathelicidins induce a cytokine called interleukin 8, which recruits and activates polymorphonuclear leukocytes. In fact, these leukocytes are the predominant cells in the pustules that characterize the papulopustular form of rosacea. That cathelicidins are elevated in rosacea was a hypothesis that has since been validated. They also enhance angiogenesis, another problem in rosacea. The good news is that everyone has cathelicidins but not everyone has rosacea. The central problem is that this “auto alarm” system of protection becomes out of balance in the skin of a rosacea prone individual.

It is useful to consider the function of the so called “skin barrier”. The Stratum Corneum is the outermost of the 5 layers of the epidermis and is largely responsible for the vital barrier function of the skin. Several researchers and clinicians over the past 5 to 6 decades have contributed to a wide body of published knowledge that supports our current understanding of how the Stratum Corneum, once thought to be biologically inert, actively contributes to the physiological homeostasis of skin. The purpose of the Stratum Corneum is to form a barrier to protect underlying tissue from infection, dehydration, chemicals and mechanical stress. Additionally, research continues to uncover specific abnormalities of the Stratum Corneum that contribute to certain dermatological diseases and/or impaired epidermal functions, and how some topical and systemic therapies may adversely affect Stratum Corneum integrity and function.
